Registration
Welcome to our on-line registration process. You may register here for a telehealth visit, well visit, sick visit, an injury, or in-office or Covid-19 testing. We highly encourage all of our patients to use our new electronic on-line registration process for any type of visit.
This process saves you time in our waiting room and makes your visit to our clinic much more efficient. If you are not able to register online you may download the paper registration forms, fill and print them out and bring them with you to your visit.
Have your information ready. Have a list of all your current medications (including dosages), medical problems, and surgeries. If you do not know your medications please contact your pharmacy for a current list. Also, have your insurance card and picture id available. If no insurance card, please have a copy of the front and back of the card.

COVID19 Testing
If you suspect that you have Covid-19 or would like testing, please e-register to get in line for testing. Patients will be seen in the clinic for illness evaluation and prescriptions if needed. Stay in your car when you arrive at the clinic and give us a call at +1 601-602-2014
Self-Pay
No insurance or high deductible insurance plan? No problem! We have a self-pay rate of $150 which covers most routine visits to our clinic, including x-rays, rapid tests, and injections. If we feel you would benefit from a procedure such as stitches or additional lab work, we will give you an estimated cost for your approval. For your benefit, we have negotiated a lower price for labs that must be sent to a reference laboratory.
Return visits for the same problem within the next six (6) days after your initial visit will not incur an additional office visit fee. Please note that the self-pay payment is due at the time of service.
